Faith Formation Registration

                 Registration is easy!  Simply click on the links on this page and print out the registration forms (these are Adobe PDF formatted.  If you need Adobe Reader click HERE).

                 Once you’ve printed the forms, fill them out and bring or mail them to Denise Sinatra at the parish office along with the $20.00 per child fee (Fees are waived for parents who volunteer their time in their child’s classroom).
